Academic Misconduct - Faculty Information
UFV is committed to maintaining and promoting instructional excellence and learning. Among the cornerstones of this commitment are academic integrity, and a mutual respect for all members of the university community. UFV students are expected to conduct themselves in a mature and responsible manner, and will be held responsible for their actions, whether acting alone or in a group.
Information for Faculty, Department Heads and Deans
When an instructor suspects or has evidence of academic misconduct, the first step is to review the Student Academic Conduct Policy (70). The policy describes academic misconduct and outlines the procedural steps in place to address student conduct violations. As an instructor, you are required to file a report (aka 'Notice of Disciplinary Action') when there is evidence of academic misconduct.

Many students have very similar or identical names. Check your class list to be sure you have the correct student.

To ensure that you have identified the correct student, include the student number as well.

If the offense did not occur in a course, please refer the matter to the Coordinator, Office for Academic Integrity and Appeals, or to your Dean.

It is important to be as detailed as possible in the evidence section. Describe the misconduct incident with specific details, including the evidence for plagiarism or cheating or other forms of misconduct.

To find out if a student has previous offenses, call the Coordinator at local 6314, and explain that you are investigating a case of academic or personal misconduct.

You may also email an enquiry, but do so via your UFV email account so your identity is email verifiable.

The Student Conduct Registry contains the names of students who have been cited for misconduct. If the student in question is in the registry, a more severe penalty for the current misconduct may be warranted. In this case, take the issue to your Dean to discuss the appropriate penalty to impose.

All misconduct and any disciplinary action must be reported in the Student Conduct Registry. Sometimes this report is the only penalty imposed. for the range of other options, please refer to the UFV Student Conduct Policy (310.12). If you plan to take more severe action than assigning a grade of zero for an assignment or exam, consult with your Dean.

If a decision is appealed, it is essential that all supporting documents be available, including your notes on the case details, names of witnesses, and so on. It is important that all of this information be maintained in secured storage in the Office for Academic Integrity and Appeals, not at the departmental level.

Send the original Notice of Disciplinary Action and a copy of the student Conduct Policy (310.12) to the student by registered mail, courier, or hand deliver it in front of a witness.

The date the student receives the notice is important to the appeal timeline if the student chooses to appeal the misconduct offense.

For this reason, if you hand deliver the notice, please have it witnessed by your Department Head or another UFV employee (i.e., Department Assistant) for verification purposes. After it is hand delivered, send a copy of the front page of the Notice of Disciplinary Action and all relevant documents to the Coordinator.

If you prefer to send the form and all relevant documents by registered mail, the Coordinator can do that for you and will add the registration receipt to the file.

Clearly print your name, sign the form, and provide today's date.

The notice will remain in the Student Conduct Registry for two years.
