Below are all relevant and addtional documents needed for submitting an application to our programs. Click on the link and print out a copy to either submit or refer to.

If you are submitting an application, please make sure that you fill out all of the required documents. A letter from O'Reg will be sent to you if your application is incomplete.

Apply online through the Admissions website. All applicants need to apply online out in order to complete their application, it will be matched with your additional documents. Additional documents (i.e. Additional Info Sheet, References & Personal Statement) can be mailed or delivered in-person to the following address:
