This form will help faculty place you in your practicum. It is very important that you answer each question carefully, knowing that you need to place a regular and important commitment of 10 to 12 hours into your weekly schedule for the next 25 to 30 weeks. By telling us what you need and what fits into your life, we can provide you with an experience that is valuable and manageable.

The theme of the 310 practicum is that it is a "stretch" practicum. That means it should be in a CYC setting that is very different from any of your previous experience. It could be a different setting, or a different age group, or perhaps a different set of clients. With that theme in mind, please answer the following questions:

It is very important for students to understand that they need to adjust their daily schedules to make their practicum work. Also, students are responsible to provide their own transportation to their practicum site.
