How to report an incident

If you are a student wanting to share an experience of sexualized violence to get support or information, please use the Disclose sexualized violence form.

Anytime an Incident Report is submitted, the Student Rights and Responsibilities Office (SRRO) will review the case to see if any UFV policy was violated. Please use the Report an incident form if you wish to report an incident of concerning or disruptive behaviour involving a UFV student(s).

Who can use this form?

UFV employees or students can use this form to report incidents of student misconduct related to:

If the incident involves UFV staff or faculty, please contact the university’s Human Rights & Conflict Resolution Office (HRCRO) instead.

What can I report to the SRRO?

The SRRO addresses student behaviour that conflicts with UFV community values, specifically under Policy 204. To determine if you should submit a report, consider the following questions:

  1. Was discrimination, bullying, or harassment involved?
    1. Policy 18 - Discrimination, Bullying and Harassment Prevention covers behaviours related to discrimination, bullying, or harassment. See Policy 18 for definitions related to this.
  2. Did the incident involve disruptive behaviour?
    1. Policy 204 -Safe Student Learning Community outlines the standards for maintaining a safe and respectful learning environment. See here for a list of prohibited behaviours.
  3. Did any form of sexualized violence occur?
    1. Policy 236 – Prevention, Education and Response to Sexualized Violence addresses the prevention of sexualized violence and provides examples of behaviours that violate this standard. See Policy 236 for a definition of sexualized violence.

If you answered yes to any of these questions, we encourage you to Report the Incident.

What information do I share?

When filling out the online form, in the incident description box please include:

  • What happened: What did you witness or experience? Describe some of the actions or behaviours that were involved. How has this impacted you?
  • Who was Involved: Share names, roles, and any identifying features of those involved, if known. Who were the individuals or groups involved in the incident?
  • Outcome: What are you hoping for by submitting a report?

The SRRO may follow up with you for additional details to assess if the behaviours fall within our policy scope.

Who will see the form?

Your report is confidential and will only be viewed by the SRRO and, if necessary, other UFV administrators. 

What happens after I submit an incident form?

After your report is submitted:

  1. An SRRO staff member will review it and reach out to guide you through the next steps.
  2. If the case proceeds to a student conduct process, a Student Support Coordinator will be assigned to provide resources and support throughout the process.
