If you are a student reporting sexual violence, please use the Disclose sexualized violence form.

Please use the Report an incident form if you wish to report an incident of concerning or disruptive behaviour regarding a UFV student or students.

Who can use this form?

UFV employees or students can use this form to report incidents regarding Safe Student Learning Community - Policy 204, or the Discrimination, Bullying & Harassment Prevention - Policy 18.

Who will see the form?

Your report is confidential and will be sent to the most appropriate person/office once received.

What happens after I submit an incident form?

After your report is submitted, a staff member in the Student Rights and Responsibilities Office (SRRO) will review it and be in touch to describe next steps and support you through the process.

Good to know:

  • Please be aware that we do not accept anonymous reports.
  • This form is for reporting alleged violations of UFV policy only. If you are reporting alleged criminal activity, also please make a report to the police of local jurisdiction.
