Certified Dental Assistant Spring Preventive Public Clinic
UFV’s Dental Clinic offers preventive dental care to clients. Our Certified Dental Assistant students provide high quality, professional, preventive care at low cost. Our large, modern clinic is equipped with the most current equipment and technology.
Open annually in May
Each spring, the Certified Dental Assistant preventive clinic offers affordable preventive services to children and young adults. Care is provided by Certified Dental Assistant students under the supervision of qualified faculty. Preventive care appointments are 2-2.5 hours in length and are generally held Tuesdays and Thursdays in May.

Fees
Client Fees inclusive of all services is $10.

Client Eligibility
In order to be eligible for services in our dental clinic, all clients must first be screened by a dentist. The completed Screening Form is then brought to your UFV Dental Clinic appointment.
Please note, prescribed treatment must be completed within 60 days of initial dentist screening.
Screening forms can be printed from our website and taken to your dentist. Forms are also available at the Health Sciences Reception desk (UFV Chilliwack campus, Building A second floor).
- 2023 UFV Dental Treatment Screening Form
- 2023 Adult Consent and Waiver Form (over age 16)
- 2023 Child Consent Form (under age 16)
- 2023 CDA Medical History Form
Clients must also be calculus tartar free (received scaling by a dentist or dental hygienist) prior to appointments at UFV.
Important to Note:
- Please keep your appointments as our students depend on your commitment.
- Records are used for teaching purposes and become the property of the University of the Fraser Valley.
- All X-rays and a copy of your treatment record will be sent to your dentist.
