Gather documentation

Before you can receive academic accommodations at UFV, you'll need to:

  • Provide documentation that tells us about your disability.

What is a disability?

The term disability covers a broad range and degree of conditions that can be permanent, temporary, episodic, and suspected.

For the purposes of our office, disability is defined as a functional limitation or impairment related to a student's ability to perform daily activities necessary to participate in post-secondary studies.

 

Requirements for your documentation

Documentation submitted to the CAS should:

1. Be completed by a relevant, qualified, and certified medical professional. 

What do we mean by relevant? 

Relevant means there needs to be a connection between the limitation and the professional providing the documentation. For example, an ophthalmologist or optometrist can provide documentation for a visual condition, but not for a mental health diagnosis.

2. Provide current information about barriers and impacts of your disability.

What do we mean by current? 

Current means the documentation must reflect the symptoms and levels of impairment you presently experience as a student.

3. Include information that is detailed and relevant to accommodation planning.

What do we mean by detailed?  

Detailed means the documentation must include a description of the functional limitation(s) or impairment(s) related to your ability to perform the daily activities necessary to participate in post-secondary studies.

 

What if your documentation doesn't meet the requirements?

Feel free to bring the documentation you have to your intake meeting. Your accessibility advisor will be happy to review it with you.

If you do not have current documentation, the Appendix 8 form available from StudentAid BC may be useful. Please discuss this with your accessibility advisor at your intake appointment.

Other Relevant Information 

The Centre for Accessibility Services will use your documentation to recommend specific academic accommodations.

The information outlined above represents UFV's internal documentation requirements which may differ from the documentation required to access external supports and services through StudentAid BC and other resources. Students should refer to the SABC website to determine eligibility for disability funding and support.
