Online forms for course registration
Prerequisite waiver form
Students without appropriate course prerequisites may submit a request with rationale for a pre-requisite waiver. Submit a prerequisite waiver form that will be reviewed by the School of Business.
Request for declaration form
BBA students wishing to declare a major and/or minor can submit their request for Advisor review via the Declaration request form.
Pay-Per-Credit request form
This form is for School of Business international students only who will be graduating in the next semester and require less than 12 credits to complete their program. Please submit this request a minimum of two weeks prior to your registration time.
