CYC 410 questionnaire

This form is for students to submit their practicum request for the CYC 410 practicum. Please be complete in all of your answers, as it is essential to know your needs in placing you in a meaningful practicum.

This form is designed to be submitted before the term begins, so that faculty is aware beforehand, and can begin to plan each placement carefully. For the CYC 410 practicum, students are encouraged to locate their own practicum placeement, but be sure to understand that the final decision fo placement is made by faculty.

Read each question carefully. Many questions ask for multiple answers, and they must be answered completely.

When you have answered all questions, you need to press the "Submit" button.  This will send the results to faculty.  Once your form has been received, you will be informed about your placeement early in the term, during Practicum Seminar.
