Applicants to the programs listed below must fill out the UFV online application for domestic students or international students and the form(s) applicable to their chosen program.

Bachelor of Arts in Child and Youth Care (BACYC)

Review the BACYC Application Checklist to ensure a complete application for consideration to this program.

Complete the following and email to admissions@ufv.ca:

  • BACYC Questionnaire
  • Résumé
  • Letter of intent outlining past experience and career objectives
  • Two reference letters (volunteer or work)

Bachelor of Education

Bachelor of Education Referee's Report (please use this form for all reference letters)

Certified Dental Assistant program

For admission to the Certified Dental Assistant program, complete and submit the Additional Application form (Dental).

Early Childhood Education

Review the ECE Application checklist to ensure a complete application for consideration to this program.

Health Care Assistant program

For admission to the Certified Dental Assistant program, complete and submit the Additional Application form (Health Care).

Nursing programs

Submit the filled-out pdf for admission to Nursing, Practical Nursing, or LPN Access programs

Social Work and Human Services

Visit the Social Work and Human Services How to apply section to obtain additional application forms for Bachelor of Social Work degree, Social Services diploma, Substance Abuse Counselling diploma/certificate, and Community Support Worker certificate.

Student forms

Appeal for late withdrawal

Complete the Appeal for Late Withdrawal if you wish to request a withdrawal after the last day to withdraw for a semester.

 

Cascade Cash Transfer Request

Use the Cascade Cash Transfer Request form to request funds be transferred from your student account to your campus card. Complete this form and submit by email through your myUFV student account.

Confirmation of Completion

Available to those who have completed all requirements for graduation, but who have not yet had their degrees conferred. Complete and submit the Confirmation of Completion‌. Immigration workshops will answer all your questions about study and work permit terms and conditions.

Confirmation of enrolment

Available through myUFV. Log into your account. Go to Student Information > Student Records > View/Print Enrolment Verification.

Graduation request

Complete and submit a Graduation Request‌ to the Office of the Registrar anytime during the semester in which you will complete your program requirements, but no later than April 1. Graduation Request forms will be available and accepted as of July 2nd, 2019.

Indigenous ancestry self-declaration

Students of Indigenous ancestry may choose to self-identify so that they may benefit from supportive programs and services. Visit the Indigenous Student Centre to learn more and fill out the self-identification form.

Medical certificate

The medical certificate form is required as supporting documentation for a late withdrawal and/or appeal for refund that involves medical circumstances.

Parchment reprint request

Complete and submit a Parchment Replacement Request‌ to the Office of the Registrar to request a reprint of your parchment. A $25 fee applies.

Permission to register (full form)

Download and fill out the permission to register form if you wish to: obtain permission to repeat a course a 3rd time; register late; register in more than six classes; and get permission for late registration from a department head. This form must be completed by the student and instructor/department/dean. Other permission to register forms can be located in the faculty forms section.

Personal information change

If you have legally changed your name or gender, have moved, or are planning to move, please complete and submit the personal information change form to the Office of the Registrar with photo ID.

Release of information

Use the release of information form to authorize UFV to release information about your student record to an individual or organization.

Request for concurrent studies

The request for concurrent studies form is for students who want to take one or two UFV courses while completing high school.

Request for deferred admission

Use the request for deferred admission form to request to defer their admission to a future intake.

Request for letter of permission (fillable PDF)

Complete the request for letter of permission form if you are planning to do course work at another institution to transfer back to UFV. If using the fillable form, save the form as your student ID and return it to articulation@ufv.ca from your UFV email address.

Request for non-disclosure of public information

Use  the request for non-disclosure of public information form to request that UFV does not release standard, public information about you.

Request for special admission

Complete the request for special admission form if you do not meet stated admission requirements and would like to request special consideration for admission.

Reset myUFV password

Students now have access to a self-service portal for new password creation or resets at https://password.ufv.ca. The password reset portal is available for creation/resets at any time of the day or night.

Transcript request

Use the transcript request to request a transcript.

Faculty and staff forms

Change of grade

Use the Change of Grade Form to change a student's grades if you have already submitted the grade to the Office of the Registrar.

End of term practices

Review faculty End of Term Practices‌ for the end of term, such as grade submission, exam conflict, and more.

Exam conflict booking

Fill out this online form to book an exam time if you have a student experiencing a conflict.

Incomplete grade contract

Use the Incomplete Grade Contract form to award a student additional time to complete coursework beyong the end of the semester. This form must be submitted with any grades of "I" Read the grade reporting policy for more information.

Notice of withdrawal

Use the notice of withdrawal form to withdraw students for failure to attend the first class or for missing pre/corequisites.

Download and fill out the permission to register form if you wish to grant a student permission to: repeat a course a 3rd time; register late; register in more than six classes; waive prerequisites; and more. This form must be completed by the student and instructor/department/dean. Other permission to register forms can be located through Faculty Web.

Use the scheduling request form to request that sections be created for inclusion in the timetable.
