Complete the Appeal for Late Withdrawal if you wish to request a withdrawal after the last day to withdraw for a semester.
Use the Cascade Cash Transfer Request form to request funds be transferred from your student account to your campus card. Complete this form and submit by email through your myUFV student account.
A Confirmation of Completion letter is now available as a self-serve option through your myUFV account. Simply login to your account, go to 'Student Information', 'Student Records' then 'View/Print Documents & Letters (Enrolment Verification, Credential Completion)'. You will then select the letter type from the drop down menu and click 'Continue'. This letter is an official UFV document and does not require a signature, stamp or seal.
Immigration workshops will answer all your questions about study and work permit terms and conditions.
Available through myUFV. Log into your account. Go to Student Information > Student Records > View/Print Documents & Letters (Enrolment Verification, Credential Completion).

Students of Indigenous ancestry may choose to self-identify so that they may benefit from supportive programs and services. Visit the Indigenous Student Centre to learn more and fill out the self-identification form.
The medical certificate form is required as supporting documentation for a late withdrawal and/or appeal for refund that involves medical circumstances.
Complete and submit a Parchment Replacement Request to the Office of the Registrar to request a reprint of your parchment.
Download and fill out the permission to register form if you wish to: obtain permission to repeat a course a 3rd time; register late; register in more than six classes; and get permission for late registration from a department head. This form must be completed by the student and instructor/department/dean. Other permission to register forms can be located in the faculty forms section.
If you have legally changed your name or gender, please complete and submit the Personal Information Change to the Office of the Registrar with photo ID.
Address changes can be completed through your myUFV account or by contacting the Office of the Registrar for alternate options.
Program declaration form - use this form to declare an adjustment to your current program.
Use the release of information form to authorize UFV to release information about your student record to an individual or organization.
The request for concurrent studies form is for students who want to take one or two UFV courses while completing high school.
Use the Request for Deferred Admission form to request to defer your admission to a future intake.

Complete the request for letter of permission form if you are planning to do course work at another institution to transfer back to UFV. If using the fillable form, save the form as your student ID and return it to articulation@ufv.ca from your UFV email address.
Use the request for non-disclosure of public information form to request that UFV does not release standard, public information about you.
Complete the Request for Special Admission form if you do not meet stated admission requirements and would like to request special consideration for admission.
Use this Subsequent Degree Declaration form to declare previous completion of a degree.
Use the Transcript Request to request a transcript.

Faculty requiring a synchronous final exam must complete the request form by the deadline provided or a final exam will not be scheduled or permitted.
